Introduction to URRSF

The Unified Radiology Report Scoring Framework (URRSF) is a novel, two-tiered scoring system designed to provide a comprehensive, quantitative, and flexible assessment of any radiology report, irrespective of modality or clinical scenario. It addresses the critical need for a standardized approach to evaluating report quality, impacting patient safety, operational efficiency, and medico-legal exposure.

The framework is architected to evaluate both universal and specific aspects of report quality through a modular design, ensuring that the final score reflects what is most important for a given report.

Tier 1: Foundational Quality Score (FQS)

The FQS assesses the universal attributes of report quality that apply to every report. This tier focuses primarily on the report's Communicative Efficacy and structural soundness, establishing a baseline of quality for all reports. It provides a maximum of 110 points across three key domains:

  • Domain A: Structural Integrity & Clarity
    • Report Structure & Organization
    • Language & Readability
    • Use of a Standardized Template
  • Domain B: Clinical Completeness
    • Clinical Context
    • Comparison to Priors
    • Completeness of Findings
  • Domain C: Communicative Efficacy & Actionability
    • Clarity and Conciseness of Impression
    • Actionable Recommendations
    • Critical Findings Communication
    • Patient-Centered Communication

Tier 2: Context-Specific Modules (CSM)

This tier adds a variable number of points based on modules that are "activated" by the report's specific context. This design allows the framework to be both universal and specialized, focusing on Content Accuracy and Clinical Relevance. Examples of CSMs include:

  • CSM-Modality: Specific to the imaging modality (e.g., CT, MRI, Ultrasound), including documentation of technical parameters and image quality.
  • CSM-Indication: Relevant to the clinical indication (e.g., Oncology Staging/Follow-up, Trauma, Incidental Findings), mandating adherence to relevant -RADS systems or systematic search patterns.
  • CSM-Setting: Tailored to the care setting (e.g., Emergency Department, Outpatient Screening, Inpatient Care), addressing requirements like Report Turnaround Time (RTAT) and specific reporting mandates.
