Aim: Uniform and unambiguous MRI evaluation of adnexal pathology & assigning risk for malignancy
When to do: Average risk women with no Acute Symptoms
ORADS 3
3 types of Cyst:
1. Unilocular
2. Multilocular
3. Cyst with solid component
4. Dilated Fallopian tube
1. Unilocular:
- Proteinaceous fluid
- Hemorrhagic
exclusions: < 3 cm hemorrhagic cyst in premenopausal women - ORADS-1 : Endometriotic cyst- ORADS 2
- Mucinous content
- Can have wall enhancement
2. Multilocular:
- Any type of fluid
exclusions: Fat -ORADS 2
- Septation & wall enhancement
3. Cyst with Solid tissue
Low risk time intensity curve on Dynamic MRI
exclusions: T2 dark/DWI dark solid component - ORADS 2
4. Fallopian Tube:
Non simple fluid
Thick but smooth fold or septation
Summary:
A complex unilocular cyst is ORADS -3
A multilocular cyst is O-RADS 3 or above
Exclusions:
- Hemorrhagic cyst below 3 cm in Pre-menopausal women,
- Endometriotic cyst without solid enhancing components
- Cyst with lipid
- Cyst with solid component, that is T2 W dark / DWI dark
