U    1. Using mesenteric nodularity as a term for panniculitis

  2.  Calling collapsed  urinary bladder as thick wall

  3.  Not mentioning severity - for eg mild/ moderate / severe fatty liver

 4.  Confusing perinephric edema due to ureteric obstruction as pyelonephritis
